International pet travel requirements vary by country and may change without notice. Please complete this form as soon as you know you will be traveling. Some destinations require testing, vaccinations, treatments, permits, or waiting periods that must be completed weeks or even months before travel.

11. CLIENT ACKNOWLEDGMENT

I understand that international pet travel requirements are established by the destination country and may change at any time. Requirements may also differ based on species, age, travel dates, route, transit countries, and method of transportation.

I understand that Terra Linda Veterinary Hospital will assist me in preparing the appropriate veterinary documentation based on the information available at the time of preparation. I am responsible for providing complete and accurate travel information and notifying the hospital immediately if my itinerary, airline, destination, travel date, or other travel arrangements change.

I understand that airlines may have requirements separate from government import requirements and that I am responsible for confirming my airline's requirements.

I understand that some destinations require advance vaccinations, laboratory testing, parasite treatments, import permits, waiting periods, government endorsement, or other steps. My pet's certificate cannot be completed until applicable requirements have been met.

I understand that final admission of an animal into another country is determined by the appropriate government/border authorities and cannot be guaranteed by Terra Linda Veterinary Hospital or the USDA.
